LONG TERM BOARDING (2 weeks are longer) MUST BE PAID IN ADVANCE! ANY PET(S) LEFT OVER 10 DAYS FROM THE PICKUP DATE WILL BE CONSIDERED ABANDONED, AND PLACED IN ANOTHER HOME OR FACILITY!
If for any reason, your dog or cat(s) needs veterinary care while staying with me, unless otherwise prearranged with your own veterinarian, I will use the veterinarian that services my kennel or an emergency veterinarian on call after hours. It will be your responsiblity to pay the bill!
It is my heartfelt desire for your pet(s) enjoy my farm! He or she will be treated like one of my dogs or cats! I will not be held liable for any pet's illness, injury, escape or death!

**PLEASE NOTE ANY SPECIAL INSTRUCTIONS ON THE BACK OF THIS SHEET:
FROM 295N or 295S take 64 EAST to the Bottom's Bridge exit 205, which is right pass the weight scales! Exit 205 is to the right, and when you come up the ramp you take a left on 249 > go approximately 3 miles! You'll pass on your right the fire department, Restaurant, gas station, Verizon Station, and you'll make a left right pass the Verizon Station onto 612! Go less than a mile, and we're on the left at 9400\9404, which is directly across from Deer Lake Estates! Come to the second house on the lane!
